an increase of LV work performed in a few patients; in the remainder the left ventricle performed less work. In four dogs, selective left coronary arteriography resulting in increased ventricular filling pressure, was accompanied by an increase of left ventricular end-diastolic (VED) and stroke volumes; the ejection fraction and heart rate remained unchanged. If the animal studies (increased VED accompanying increased ventricular filling pressure) can be applied to patients with heart disease, the alteration of left ventricular function can be considered on the basis of the Frank-Starling mechanism. To cope with the temporary depression of its function, the left ventricle of patients with heart disease utilized the Frank-Starling mechanism resulting in an adequate (essentially unchanged) cardiac output from an increased left ventricular filling pressure. The normal left ventricle of the dog, stressed more than is appropriate clinically, shows

Mechanisms mediating bradycardia during coronary arteriography.

Cardiac slowing occurring during diagnostic coronary arteriography was studied in 78 patients. Comparable degrees of slowing occurred with injections into the right and into the left coronary arteries into the contralateral artery, and with injections into the coronary artery giving rise to the sinus node artery and into the contralateral artery. Rapid intracoronary injections of isosmotic dext...

Is cardiac MRI adenosine stress perfusion imaging a more appropriate diagnostic tool for obese patients?

Background Cardiac nuclear stress perfusion imaging is subject to artifact related to obesity, breast attenuation and diaphragm overlap. These factors are responsible for “false positive” results among patients undergoing evaluation for coronary atherosclerosis, leading to potentially unnecessary invasive coronary arteriography. Cardiac MRI adenosine stress perfusion imaging is unaffected by bo...

Major complications of coronary arteriography: the place of cardiac surgery.

The records of the catheter laboratory at St George's Hospital between 1983-1988 were reviewed to determine how often emergency coronary bypass surgery was performed because of a complication arising during elective coronary arteriography. A total of 11,216 cardiac procedures were performed; 5781 were confined to left ventricular angiography and coronary arteriography in patients with suspected...

Incidental diagnosis of a large coronary fistula: angiographic and cardiac MRI findings.

Coronary artery fistulas are rare anomalies of the coronary arteries that may sometimes be totally asymptomatic and can be found incidentally during coronary arteriography later in adult life. We report the case of a 75-year-old man with such a fistula and describe our diagnostic approach. In addition, we review the literature on the etiology, epidemiology, the diagnostic modalities and treatme...

ACR practice guideline for the performance and interpretation of cardiac computed tomography (CT).

Cardiac computed tomography (CT) is an evolving modality that includes a variety of examinations to assess the anatomy and pathology of the cardiac chambers, valves, myocardium, coronary arteries and veins, pericardium, aortic root, and central great vessels. The development of multidetector CT scanners with increasing numbers of detector rows, narrow section thicknesses, increasing scanner spe...
